MAP Jamaica — Moving Above Parkinson's, Inc. ("MAP Jamaica," "we," "us," or "our") is a registered 501(c)(3) nonprofit organization (EIN 42-1864710), headquartered at 113 Windsong Drive, Social Circle, GA 30025, and operating programs in Jamaica and the Cayman Islands. This policy explains how we handle information collected through mapjamaica.org and related forms, surveys, and donation pages.

2. Information we collect

We collect information you provide directly to us, including:

• Contact form submissions — name, email address, organization, and message content.
• Survey responses — information you choose to share through our Cayman Islands Proof of Concept surveys, including experiences related to Parkinson's disease, caregiving, or clinical practice.
• Donations — payment and contact information processed securely by our donation platform, Zeffy. MAP Jamaica does not directly collect or store payment card information.
• NDA submissions — name, email, and signature information submitted through our confidentiality agreement form.

We do not use cookies or tracking technologies to build advertising profiles, and we do not sell personal information to third parties.

3. How we use your information

• To respond to enquiries submitted through our contact form.
• To analyze and report on aggregate survey findings as part of our Caribbean Proof of Concept research initiative.
• To process and acknowledge donations, and issue tax receipts where applicable.
• To communicate with board members, partners, and collaborators regarding confidentiality obligations under signed NDAs.
• To improve our programs, website, and community outreach.

4. Third-party services

We rely on trusted third-party platforms to operate parts of this website. Each acts as a data processor on our behalf, and MAP Jamaica retains ownership and control of the underlying data submitted through these services:

• Tally (survey forms) — Belgium/EU-based, GDPR-compliant, data stored on EU infrastructure.
• Formspree (contact form) — used to route contact form submissions to our team.
• Zeffy (donation processing) — a donor platform used to securely process and manage gifts.

We encourage you to review each provider's own privacy policy for details on how they handle data in transit and at rest.

5. How we protect your information

We take reasonable administrative and technical measures to protect the information you share with us, including relying on providers with industry-standard security practices. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security.

6. Data retention

We retain contact and survey information for as long as needed to support our research, reporting, and organizational record-keeping, or as required by law. You may request deletion of your information at any time (see Section 8).

7. Children's privacy

Our website and programs are intended for adults. We do not knowingly collect personal information from children under 13.

8. Your rights and choices

You may request access to, correction of, or deletion of your personal information held by MAP Jamaica at any time by contacting us using the details below. We will respond to verified requests within a reasonable timeframe.
